PG&E continues to struggle with flawed record keeping for its natural gas system, and is failing to learn from its experiences, five years after a fatal explosion in San Bruno, according to an audit ordered by state regulators that was circulated Thursday. The audit was conducted by Florida-based P Wood Associates, a risk management and assessment firm, and was commissioned by the state Public Utilities Commission.
