NTSB released investigative update regarding September 8, 2015 engine fire experienced by British Airways flight 2276, a Boeing 777, during takeoff at McCarran International Airport. Investigators disassembled and documented GE90 engine at GE facility in Evandale, OH. Examination revealed one portion of stage 8-10 spool in high-pressure compressor (HPC) section failed; fracture initiated in HPC stage 8 disk web. NTSB will continue metallurgical evaluations of disk and fracture features.

A Texas district judge issued temporary restraining orders on Tuesday to preserve evidence in the death of a bulldozer operator at Total SA's Port Arthur, Texas, refinery on Saturday, according to court documents. Thomas Counts, a Kinder Morgan Inc employee, drowned when the bulldozer he was operating in a coker pit at the Total refinery overturned in scalding water early on Saturday, according to law enforcement officials.

NTSB issues investigation update on Delta accident at LaGuardia

Delta Flight 1086 was coming in from Atlanta when it skidded off a runway at LaGuardia Airport around 11 a.m. on March 5. As part of its ongoing investigation into the March 5 accident at LaGuardia Airport where Delta Air Lines flight 1086 veered off the runway shortly after touching down, the National Transportation Safety Board has released a third investigative update. At about 11:02 a.m., the Boeing MD-88, flying from Hartsfield-Jackson Atlanta International Airport to New York exited the left side of runway 13, struck a perimeter fence with its left wing, and came to rest with its nose on an embankment.
