Barra on recall: GM on track to answer 'what went wrong and why'

General Motors Co. CEO Mary Barra said the Detroit automaker is still working to understand what went wrong in the company's recall of 1.62 million cars linked to 12 deaths and 31 crashes for ignition switch defects - a decade after it first learned of problems.
