NTSB says defective rail likely caused 2014 Lynchburg oil train derailment

LYNCHBURG, Va. - The derailment of a train carrying highly volatile crude oil in downtown Lynchburg two years ago would not have occurred if CSX Transportation had time to carry out the recommendations of a working group on rail failure accepted two weeks before the accident, a federal safety agency has concluded.

NTSB addresses lithium batteries as cargo on aircraft.

NTSB issued 2 safety recommendations to physically separate lithium batteries from other flammable hazardous materials stowed on cargo aircraft and to establish maximum loading density requirements that restrict quantities of lithium batteries and flammable hazardous materials. These safety recommendations, addressed to the Pipeline and Hazardous Materials Safety Administration, are derived from the investigation of the July 28, 2011, in-flight fire and crash of Asiana Airlines Flight 991.

NTSB to launch second search mission to EL FARO.

NTSB will launch a second expedition to search for evidence in its investigation of the loss of the cargo ship EL FARO, which sank in the Atlantic during a hurricane on Oct 1, 2015. Key objective of upcoming mission, which is expected to begin in April, is to locate the voyage data recorder and to provide investigators with a more extensive and detailed survey of the shipwreck. If VDR is located, another mission using a remotely operated vehicle capable of recovering recorder will be initiated.

NTSB Regulatory Changes streamline reporting requirement.

NTSB announced that its direct final rule to narrow reporting requirement in 49 C.F.R. § 830.5(a)(10) concerning certain resolution advisories is now effective. Operators are now only required to report Airborne Collision and Avoidance System resolution advisories issued when an aircraft is being operated on an instrument flight rules flight plan and compliance with the advisory is necessary to avert a substantial risk of collision between two or more aircraft.

NTSB announces second search mission for El Faro voyage data recorder

The US National Transportation Safety Board (NTSB) has announced a second search mission to locate the voyage data recorder (VDR) of TOTE Maritime's cargo ship El Faro, which disappeared during Hurricane Joaquin on 1 October.
