NTSB releases final report on crash of de Havilland DHC-3.

According to the NTSB, probable cause for the crash of a de Havilland DHC-3 in Soldotna, Alaska, on July 7, 2013, was the operator’s failure to determine actual cargo weight, leading to loading and operation of airplane outside of its weight and center of gravity limits. Contributing to the accident was FAA's failure to require weight and balance documentation for this type of air taxi flight. Flight was headed from Soldotna to a lodge 90 miles away when airplane stalled and crashed on takeoff.

NTSB updates investigation into sinking of cargo ship.

In its continuing investigation of the sinking of the cargo ship El Faro in the Atlantic Ocean near the Bahamas, NTSB has found that on February 13, 2015, El Faro successfully completed the American Bureau of Shipping class and statutory surveys. Annual inspection of El Faro, required by the USCG, was completed by qualified USCG inspectors in San Juan, Puerto Rico, on March 6, 2015. Vessel’s terminal manager reported that El Faro met stability criteria when it left Jacksonville.

NTSB Issues Second Update on British Airways engine fire.

NTSB released investigative update regarding September 8, 2015 engine fire experienced by British Airways flight 2276, a Boeing 777, during takeoff at McCarran International Airport. Investigators disassembled and documented GE90 engine at GE facility in Evandale, OH. Examination revealed one portion of stage 8-10 spool in high-pressure compressor (HPC) section failed; fracture initiated in HPC stage 8 disk web. NTSB will continue metallurgical evaluations of disk and fracture features.

NTSB calls for direct federal safety oversight of WMATA.

NTSB issued two urgent safety recommendations calling for direct federal safety oversight of Washington's Metrorail system by the Federal Railroad Administration. In its safety recommendation letter to the Secretary of Transportation, NTSB said that in the past 33 years, it had launched 11 investigations of WMATA rail accidents that had killed 18 people. Many of those investigations involved WMATA’s inadequate management of safety operations.
