NTSB Recommendation addresses Washington's Metro hazards.

In its ongoing investigation of Metro train accident at L’Enfant Plaza on January 12, 2015, NTSB investigators found electrical connections in Washington's Metro system require immediate action. Construction and installation of certain electrical connections and assemblies associated with power supply to third rail were identified as missing sealing sleeves designed to keep out moisture and contaminants. NTSB asked WMATA to develop and implement program to rectify these hazards.

In Special Investigation Report, The Use of Forward Collision Avoidance Systems to Prevent and Mitigate Rear-End Crashes, NTSB stresses that collision avoidance systems can prevent or lessen the severity of rear-end crashes, saving lives and reducing injuries. NTSB recommended that the technology become standard on all passenger and commercial vehicles. Report notes that lack of incentives and limited public awareness has stunted wide adoption of collision avoidance technology.

NTSB cited human factors as probable cause in UPS crash

Human factors including fatigue and the failure to properly configure a flight computer were the probable cause in the 2013 crash of a United Parcel Service Inc cargo jet in Birmingham, Alabama, the National Transportation Safety Board said on Thursday.
